Diabetes Mellitus Induced by a Rodenticidal Drug , RH-787 ( N-3-pyridylmlthyl-N`-p-nitrophenylurea )

Abstract

Autopsy result of a drug induced diabetes mellitus case with clinical features of glycosuria, proteinuria, hemoglobinuria and coma were presented. The patient was a 41-year-old Korean female, who took rodenticidal drug, RH-787 for suicidal purpose. About six days later, diabetic ketoacidosis was found and 10 days later, sie died of bronchopenumonia.
In pancreas massive destruction of Langerhan´s islet cell was found under the microscope. Vacuoles in proximal tublar epithelium of kidney and compatible findings of early acute tubular necros is were also found.
Diabetes mellitus induced by RH 787 was certainly due to massive necrosis of pancreatic islet cells. Three phases of course usually, found in alloxan´s diabetes mellitus; i,e, initia hyperglycemia, hypoglycemia and permanent diabetes mellitus, seem to be present in RH-787 induced diabetes mellitus. Nephrotoxicity is also inemspected. Mechanism of action, metabolism and treatment of RH-787 toxicity should be elucidated in future.
Clinical features of the 9 cases of RH-787 intoxication was also presented.
